To fill in the V55/5 there are two forms currently in use using the date codes 9/05 and 2/10 under the V55/5 form number in the top right hand corner. If you go to your lo the local DVLA office or send off to obtain a V55/5 it will be the later 2/10 however there are still stocks of the older form about. They are gradually being phased out and will cease to be valid soon.
If it is 9/05 then use the directions below:-
To fill in the V55/5 the following boxes need to be filled in.
1. Question 1 ‘the registration mark’
2. Question 2 Taxation Class ‘Historic or unlicensed’
3. Question 3 Leave blank
4. Question 4 Leave blank
5. Question 5 Make ‘BSA’
6. Question 6 Model the model of your machine e.g. ‘Golden Flash, B31,       C11G’ etc.
7. Question 7 Motorcycle
8. Question 8 2 wheel
9. Question 9 Colour main tank colour e.g. Gold, Green, Maroon etc
10. Questions 10-24 N/A
11. Question 24 and 25 Only if you have a log book then take the date of         first registration e.g. 23rd of August 1950 would be 230850         otherwise leave blank
12. Question 27 N/A
13. Question 28 for the V765 process N/A
14. Question 29 Petrol
15. Question 30 The full frame number (take it off the machine and not        off the log book)
16. Question 31 The full engine number (take it off the machine and not        off the log book)
17. Question 32 The cylinder capacity.
18. Questions 33-41 N/A
19. Question 42 Year of manufacture only if you know it.
